  2. Then you’ll have to scroll down and choose the “download forms” option from the documents column at the bottom.
  3. You can also choose the “registration of births and deaths” option from the documents column. It will take you to a new page where you’ll have to choose the “forms” option.

  3. As soon as possible from the borough authorities if the child was delivered at a public place or home. The complete event should be reported to the concerned health care unit of the municipality/ward/panchayat by the head of the family.
