How to apply for a heavy driver’s license offline

Anyone who wants to get a heavy driving license can also apply for the license offline. To do so, he/she must follow the steps mentioned below: 

•  Go to the nearest RTO office and get Form 2. 
•  Complete Form 1A which contains the certificate of your medical. 
•  Complete Form 5, where the applicant can certify that he or she has completed the required training at a state or government-affiliated automotive school. 
• Submit Form 2, Form 1A and Form 5, along with relevant documents such as  proof of address, proof of identity, and passport size photograph.
