Apply for Passport in Kavaratti

For new/fresh passport application under any category, the mandatory documents required for verification are as:

1. Proof of Address
2. Proof of Age

As per the category of applicant, the additional documents required for documents verification are as:

Minor
If applicant is below 18 years of age.

3. A declaration affirming the details filled in the application form about the minor as per Annexure D

Proof of Present Address

For Proof of address, you can submit following documents:

1. Photo Passbook of running bank account in any Indian scheduled public sector bank, private sector bank and regional rural banks
2. Water bill
3. Election Photo ID card
4. Landline or Post-paid mobile bill
5. Proof of Gas Connection
6. Election commission Photo ID card
7. Spouse's passport copy (First and last page of the passport that includes the details of the family and mentions applicants name as the spouse of the passport holder)
8. Certificate from Employer of reputed companies on letter head
9. Income Tax Assessment Order
10. Aadhaar card
11. Electricity Bill
12. Rent agreement

Proof of Age (Date of Birth)

For Proof of Age or Date of Birth, you can submit following documents:

1. School leaving/Matriculation Certificate (SSC/10 class) issued by the school last attended/recognised educational board
2. Driving Licence
3. PAN Card
4. Election commission Photo ID card
5. Aadhaar card or E-Aadhaar
6. Birth certificate issued by the Municipal Corporation, the Registrar of Births and Deaths, or by any other designated authority authorised by the Registration of Births and Deaths Act, 1969.
7. Declaration given by the Head of the Orphanage/Child Care Home on the official letterhead confirming the date of birth of the applicant.
8. Policy Bond issued by the Public Life Insurance Corporations/Companies which contains the date of birth of the holder.
9. Extract of the service record of the applicant (in case of government servants) or Pay Pension Order (retired government servants) that is duly attested/certified by the officer/in-charge of the Administration of the concerned Ministry/Department of the applicant.
