Documents required for lost passport 

Documents required for lost passport are same for adults and minors. Lost passport application cannot be processed under Tatkaal scheme. You need these following documents to apply for lost passport:

1. Date of Birth Proof
2. Affidavit informing how and where the passport was lost as per Annexure “F”
3. Original Police report
4. Supporting documents for Non-ECR category
5. Photocopy of first two and last two pages of Passport.

Date of birth proof for lost passport application

Below is the list of documents acceptable for date of birth proof in the process of Adult Fresh passport application: 

1. Birth Certificate issued by the Municipal Corporation or other authority
2. Transfer/School leaving/Matriculation certificate
3. Policy bond by life insurance corporation with applicant’s date of birth
4. Service Record/pay pension order and other government related document
5. Voter ID
6. PAN card
7. Driving Licence
8. A Declaration given by the head of the orphan/child health care.

Below is the list of documents acceptable for address proof in the process of Adult Fresh passport application: 

1. Water Bill
2. Telephone Bill
3. Income Tax Assessment Order
4. Electricity Bill
5. Photo ID Card
6. Proof of Gas Connection
7. Certificate from Employer or Company Letterhead
8. Spouse Passport Copy
9. Aadhaar Card
10. Rent Agreement
11. Photocopy of Active Bank Account Passbook

Steps for lost passport application in India

1. Report loss immediately – If you are in India, report loss at nearest police station as soon as possible. If you are abroad, report loss at nearest Indian Mission.
2. Apply for emergency certificate if urgent – If there is emergency like illness or death in family, contact Indian Mission. After verification, they may issue emergency certificate so you can travel back to India.
3. Apply under “Reissue” category – You must apply for lost passport under “Reissue” option on passport portal. Fill form carefully. Pay required fees as per your category. Take printout of application form.
4. Collect required documents – Keep all required documents ready for lost passport under “Reissue” category.
5. Visit Passport Seva Kendra – Submit application form and documents at Passport Seva Kendra. Complete biometric and verification process. After this, police verification may take place. Once approved, you will receive your new passport.
