8. Policy Bond issued by the Public Life Insurance Corporations/Companies which contains the date of birth of the holder.
9. Extract of the service record of the applicant (in case of government servants) or Pay Pension Order (retired government servants) that is duly attested/certified by the officer/in-charge of the Administration of the concerned Ministry/Department of the applicant.

How to Apply for Passport in Chennai

Here are the steps to apply for a fresh passport through the Passport Seva Online Portal:

1. Go to the Passport Seva Online Portal and click the “Register Now” link on the home page to create an account.
2. Login to the portal using the registered Login ID.
3. Click the “Apply for Fresh Passport” link.
4. Choose any one of the methods of application as given

(a) Download E form and upload later: Download e- form, fill it and upload the filled e-form on same page
(b) Filling online application form: Click the given link of passport application online form, fill the form with required details and submit it.

5. Once the application is submitted, schedule an appointment and click the “Pay and Schedule Appointment” link on the “View Saved/Submitted Applications” screen.
6. Select PSK or POPSK, check for earliest appointment date available and Click on Pay and Book Appointment.
7. Online payment is mandatory for booking appointments at all PSK/POPSK/PO. Choose the payment modes: Credit/Debit Card, Internet Banking or SBI Bank Challan.
8. After making the payment, click the “Print Application Receipt” link to print the application receipt containing the Application Reference Number (ARN)/Appointment Number.
9. Visit the Passport Seva Kendra (PSK)/Regional Passport Office (RPO) on your appointment date, time with the original documents.
10. Verification of Address by local police
11. Passport dispatch to given address by post
