1. Form no. 1:
Form no. 1 is used for the first application of driving licence. This form is called Application cum declaration of physical fitness. Applicants must declare whether they have any disease or disability that may affect their driving ability.
2. Form no. 1A:
Form no. 1A is medical certificate. Used to medically verify the details provided in Form 1. This form must be filled and signed by registered medical practitioner
3. Form no. 20:
This form 20 comes under vehicle registration. Applicants must fill this form to register new motor vehicle. This includes details like vehicle type, fuel type, engine details and seating capacity.
4. Form no. 26:
Form no. 26 is used for the duplicate certificate of registration. If the applicant lost his original registration certificate then you have to fill out this form to apply for duplicate registration certificate.
5. Form no. 27:
Form no. 27 is used for re registration of vehicle. Required when vehicle owner moves from one state to another and needs a new registration number.
6. Form no. 28:
Form no. 28 is no objection certificate. You will need this when transferring vehicle to another state or RTO.
