Difference between medical certificate in and form 1-A
Before you understand process, you should know difference between Medical Certificate and RTO Form 1A. Both are related to your health, but they are not same.
Medical certificate – Doctor gives you medical certificate about your health. It confirms you are fit to drive. You need it when:
- You are above 40 years and want to renew license
- You have health conditions like epilepsy or diabetes
Form 1A – Form 1A is required for driving license application. It records your medical fitness. You do not need separate medical certificate to fill Form 1A. But if you are above 40 years, you must submit both Form 1A and medical certificate.
It’s important to submit driving license renewal medical form or form 1A to the RTO.
Driving license renewal medical form
Follow these steps to get fitness certificate for RTO:
- Download Form 1A: Visit Parivahan Sewa website. Go to Informational Services > Downloadable Forms
- Visit registered doctor: Take printout to doctor with MBBS degree. Doctor must be approved by local RTO
- Medical check-up: Doctor will check vision, hearing, reflexes
- Doctor’s sign and stamp: After check-up, doctor signs form and adds official stamp
- Upload form: Scan signed Form 1A and upload PDF during online application or renewal on Parivahan portal
Form 1A medical certificate declarations
Following are the declarations that are stated in form 1A medical certificate:
- Declaration 1a: If there is any eyesight problem, doctor must confirm if it can be corrected with glasses.
- Declaration 1b: It states if you can see colors like red and green
- Declaration 1c: Confirms ability to see vehicles and number plates from at least 25 meters
- Declaration 1d : It verifies if you can hear things or are you deaf? If yes, does it interfere with how you drive a car?
- Declaration 1e: It states that if you have night blindness.
- Declaration 1f: Do you have any defects or missing limbs that would interfere with how you drive a car?
- Declaration 1g: (Optional): Records blood group and Rh factor
Medical certificate necessary for a learning license or not
Medical certificate is not always required for learner license. It depends on age, vehicle type and RTO rules. Here are cases where medical certificate is required:
- Age above 50 years
- Apply for heavy or transport vehicle license (truck, bus, taxi)
- State RTO rule (some states ask even for learner license)
For normal learning license under 50 years, medical certificate is not required. But check local RTO rules, because some states make it compulsory.
Who signs Medical Certificate for Driving License Renewal
Medical certificate can be issued by government doctor or registered medical practitioner (MBBS). Doctor must be authorized as per RTO rules. The RTO office does now no longer gives driving license renewal medical certificate from Ayurvedic practitioner. Authorized doctor appointed by state government or approved by RTO can sign Form 1A.
