- If minor travels alone: written consent from parents/guardians certified by public notary, copies of parents’ passport or ID proof
- Application form: parents or guardians must accompany minor to sign, except when minor is visiting parent in New Zealand
Married Applicants
- If marital status not shown in passport, submit marriage certificate
