Forms that are required at the RTO Office Nashik

For any service at Nashik RTO, you need to fill the related form. Here are some commonly used forms:

1. Form no. 1:

Form no. 1 is first application form for driving licence. This form knows as application cum declaration of physical fitness. You must fill out this form to declare if you have any disease or disability that affects your driving skill.

2. Form no. 1A:

Form no. 1A  is medical certificate. It is used to verify information given in Form No. 1.

3. Form no. 20:

This form 20 is used for vehicle registration. This form is used for the application for registration of motor vehicle. In this form, you need to mention the details of vehicle such as type of vehicle, fuel engine used in the vehicle, seating capacity, etc.

4. Form no. 26:

Form no. 26 is used to apply for duplicate registration certificate. If original RC is lost or damaged, this form is required.

5. Form no. 27:

Form no. 27 is used to apply for new registration number. This form is required when vehicle is moved from one state to another.

6. Form no. 28:

Form no. 28 is application for No Objection Certificate (NOC). You use this form to get NOC required for driving license.

7. Form no. 30:

You can use form no. 30  to transfer vehicle ownership. You have to submit it when you sell or transfer a vehicle to another person. You must mention details of transferor and transferee.

8. Form no. 31:

You can use form 31 when the vehicle owner has passed away. A legal heir uses this form to transfer vehicle ownership.
