A medical certificate for a driving license is a document issued by a qualified medical professional. They certify that you are physically and medically fit to drive a vehicle. A medical fitness certificate is one of the key requirements for a new DL or renewing your driving license after the age of 40 years. In this blog, we’ll cover everything you need to know about the medical fitness certificate for a driving license and the required Form 1A.

Who needs medical fitness certificate for driving licence

A medical fitness certificate is required to apply for a permanent driving license and for DL renewal if you are :

  • Older than 40 years
  • Applying for a commercial driving license (e.g., truck, bus, taxi)
  • Having certain medical conditions (e.g., vision issues, neurological disorders)

Form 1 Vs Form 1A: What’s the Difference

There are two types of medical forms that will be required for the driving license application.

Form 1

Form 1 is Application-cum-Declaration for Physical Fitness. It is a self-declaration medical form for a driving licence.

Every driving license applicant needs to fill out the form online and submit it while applying online.

Form 1A

Form 1A, a medical form for a driving license, is required for those above 40 years of age. It needs to be submitted along with other documents. The form is to be filled out and attested by a Ghazetted government hospital doctor or CMO. It also comprises a medical fitness certificate for a driving license.

Medical certificate for driving licence renewal

If you are over 40 years of age or applying to renew a commercial/transport vehicle licence, you will need to submit a medical certificate for driving licence renewal. You have to provide Form 1A signed by a registered medical practitioner.

The primary purpose of a medical certificate is to ensure that all drivers meet the minimum health standards necessary to operate vehicles safely, thereby reducing the risk of accidents on the road.

Who can sign a medical certificate

  • Only a registered medical practitioner with an MBBS degree or higher can issue this certificate.
  • The doctor must be approved by your regional RTO.

The steps to get a medical certificate for renewal of a driving licence are the same as for a new DL.

Form 1A medical certificate declarations

The following are the declarations that are stated in Form 1A medical certificate:

1) Declaration 1a: If you have any eyesight-related issues. If yes, then the doctor has to mention that it can be fixed with glasses

2) Declaration 1b: It states that you can see colors like red and green

3) Declaration 1c: It states that you can see cars and number plates from a distance of at least 25 meters

4) Declaration 1d: It verifies if you can hear things or if you are deaf. If yes, does it interfere with how you drive a car?

5) Declaration 1e: It states that if you have night blindness.

6) Declaration 1f:  Do you have any defects or missing limbs that would interfere with how you drive a car?

7) Declaration 1g: (Optional)  You have to tell your blood group and Rh factor

Who must submit a medical certificate for a driving licence

1. Applicants above 40 years of age

If you’re 40 years or older, you must submit a medical fitness certificate when applying for a new licence or renewing an existing one. This rule ensures that age-related health concerns like poor eyesight, slower reflexes, or chronic conditions are monitored. For example, if a 45-year-old is renewing their two-wheeler licence, they will need to provide a Form 1A signed by a registered medical practitioner.

2. Applicants for a commercial driving licence

Driving a commercial vehicle—like a truck, bus, taxi, or auto-rickshaw—requires more responsibility. Therefore, anyone applying for a commercial driving licence must submit a medical fitness certificate regardless of age. For instance, even a 25-year-old interested in driving an Uber or goods vehicle must undergo a medical checkup and submit the certificate.

3. Individuals with known medical conditions

If you have any pre-existing medical issues such as epilepsy, diabetes, vision problems, or heart conditions, a medical certificate is necessary—even if you’re under 40 or not applying for a commercial licence. For example, if a 30-year-old with controlled epilepsy is applying for a learner’s licence, the Regional Transport Office (RTO) will require a medical certificate to ensure they’re fit to drive safely.

4. Applicants for a learner’s licence in certain states

While not always required, some Indian states may ask for a medical certificate even at the learner’s licence stage, especially for commercial or transport category licences. It’s always best to check with the local RTO.

What Tests Are Conducted in RTO Medical Examination?

During Form 1A examination, registered medical practitioner checks applicant’s vision, hearing, physical fitness and medical history to assess fitness to drive.

1. Vision and Eye Test

  • Visual Acuity: Checks distance vision at 25 metres in daylight, with or without glasses.
  • Colour Vision: Checks ability to identify colours using Ishihara charts, especially colours used in traffic signals.
  • Night Vision: Checks vision in low light and ability to see objects from the sides.

2. Physical Fitness and Reflexes

  • Hearing: Checks whether applicant can hear normal sounds and respond to signals while driving.
  • Limb Movement: Checks movement and control of hands, arms and legs needed to operate steering, brakes and gears.
  • Motor Coordination: Checks balance, body movement and basic coordination.

3. Medical History Check

  • Doctor reviews medical history and checks for conditions such as epilepsy, vertigo, serious heart problems and sudden loss of consciousness.
  • These conditions are assessed to see whether they can affect safe driving.

4. Blood Group Details (Optional)

These details may be printed on physical driving licence for identification during medical emergencies

Applicant may provide blood group and RH factor details for recording on Form 1A.
