Documents required for new passport

List of documents that needs to be submitted in order to apply for a General Passport under different categories

Address proof– One of the following documents can be submitted as address proof for Passport.

  • Election ID card with present address
  • Aadhar card
  • Phone bill with current address
  • Gas connection
  • Bank passbook
  • Rental agreement if staying in a rented house
  • Income Tax Assessment order
  • Water bill with current address
  • Electricity bill with current address
  • Certificate from employer on letterhead with a seal and signature
  • Passport of the spouse
  • Parents’ passport

Age proof– One can submit any of the following documents as age proof for passport:

  • Birth certificate issued by the municipal authority or any other office authorized to issue the birth certificate
  • School leaving certificate/ Secondary school leaving certificate
  • Certificate sworn before a magistrate

Documents required for Tatkal Passport

Listed below are the documents required to apply for a Tatkal  passport –

Address proof– One of the following documents can be submitted as address proof for Passport.

  • Election ID card with present address
  • Aadhar card
  • Phone bill with current address
  • Gas connection
  • Bank passbook
  • Rental agreement if staying in a rented house
  • Income Tax Assessment order
  • Water bill with current address
  • Electricity bill with current address
  • Certificate from employer on letterhead with a seal and signature
  • Passport of the spouse
  • Parents’ passport

Age proof–  Any of the following documents can be submitted as proof of age for passport:

  • Birth certificate issued by the municipal authority or any other office authorized to issue the birth certificate
  • School leaving certificate/ Secondary school leaving certificate
  • Certificate sworn before a magistrate

Documents required for Diplomatic passport

In order to apply for an Official or Diplomatic Passport , the following is a list of documents that will have to be provided. One copy of the documents mentioned below will have to be submitted.

  • Official identity card of the applicant or that of the Head of Office
  • A certificate that has been issued by the Head of Office
  • An Official Request Letter from the Forwarding Officer
  • PMO or Political Clearance Certificate, if needed
  • For existing passport –
    • Official or Diplomatic Passport (in reissue applications) – Original Diplomatic or Official Passport for cancellation. In case this has been kept in the custody of the Ministry of External Affairs, the original Safe Custody or Surrender Certificate will have to be provided. In case this has been cancelled by the Ministry of External Affairs, the Original Cancellation Certificate will have to be provided.
    • Ordinary Passport – Certificate of Safe Custody
    • In case an official is retiring is less than 6 months from the application date, he/she will have to provide an undertaking from his/her office that the Diplomatic or Official Passport will be given to the office as soon as he/she returns.

Applicants must note that the documents must be placed in the order given below –

  • Printed copy of online filled application form
  • Political or PMO clearance
  • Copy of identity card
  • Certificate from the Head of Office
  • Request from a Forwarding Officer
  • Other necessary documents

Documents required for New born baby passport

  • Parent(s) of the newborn child must first register an account with the Passport Seva Kendra if not done before.
  • Generally for minors, an appointment need not be taken, hence the family can visit the Passport Seva Kendra during working hours along with a receipt of Application.
  • Candidates must also have the following documents (original and photocopies) –
    • Birth Certificate of the baby
    • Proof of current address (if parent has a Passport then this will suffice)
    • Filled and signed Annexure H which can be procured from the Passport Seva Kendra website.
    • Passport photo of the baby against a background that is white
    • Appointment receipt
    • Marriage certificate if the spouses name has not been endorsed on a parents Passport
  • Additional verifications will be done after this for all documents.

Documents required for Minor passport

If parents wish to procure passport for a minor, the following are the documents that will have to be furnished by them –

Unless specified, it will be assumed that both parents have given their consent for the same.

  • Current address proof in the name of the parent(s) will have to be provided
  • Original and self-attested copies of the parents’ passport will have to be provided to the Passport Seva Kendra in case both parents possess the passport
  • Documents of minors will have to be attested by parents
  • Until the minor is 18 years old, he/she will be eligible for Non-ECR passport wherein Emigration check is not required
  • Original documents along with a set of self-attested photocopies of the originals will have to be furnished at the Passport Seva Kendra for processing
  • A Declaration confirming the details mentioned in the application about the minor applicant as per: Annexure ‘D’.

Documents required for renewal of minor passport

The following documents are required for renewing passport of minors:

  • Old Passport (original). Along with it self-attested copies of the following-
    • First and last 2 pages.
    • Non-ECR and ECR page.
    • Page of Observation (if any).
    • Authority and validity extension page (if any).
  • A Declaration confirming the details mentioned in the application about the minor applicant as per: Annexure ‘D’
  • Original and self-attested copies of the passport of parents

Note – Parents can attest the documents of minor applicants.

Documents required for Lost passport

  • An affidavit that gives information about where and how the passport was stolen/lost
  • Original police report
  • Self-attested copy of ECR or Non-ECR page
  • Self-attested copy of validity extension page (in case of short validity passport)
  • Address proof– One of the following documents can be submitted as address proof for Passport:
  1. Election ID card with present address
  2. Aadhar card
  3. Phone bill with current address
  4. Gas connection
  5. Bank passbook
  6. Rental agreement if staying in a rented house
  7. Income Tax Assessment order
  8. Water bill with current address
  9. Electricity bill with current address

Documents required for damaged passport

  • Original old passport.
  • Self attested copy of the first and last two pages of the passport.
  • Self attested copy of ECR or Non-ECR page.
  • Self attested copy of validity extension page (in case of short validity passport).
  • An affidavit that gives information about how the passport got damaged.
  • Age proof – The following can be submitted :
  1. Birth certificate issued by the municipal authority or any other office authorized to issue the birth certificate
  2. School leaving certificate/ Secondary school leaving certificate
  3. Certificate sworn before a magistrate

Documents required for passport-If you want to change the existing details in your Passport

  • Original old passport
  • Self attested copy of the first and last two pages of the passport
  • Self attested copy of ECR or Non-ECR page
  • Self attested copy of validity extension page (in case of short validity passport)
  • Any one of the following documents:Attested copy of marriage certificate issued by the Registrar of Marriages.
  1. A joint affidavit from wife and husband along with a photograph together.
  2. Affidavit as per Annexure K.
  3. Self attested photocopy of the spouse’s Passport.

Documents required for passport-If you are a Student applying for Passport

  • Address proof- One of the following documents can be submitted as address proof for Passport:
  1. Election ID card issued by Election Commission
  2. Aadhar card
  3. Phone bill with current address
  4. Bank passbook of an active bank account (scheduled public & private Indian sector banks and RRBs)
  5. Rental agreement (registered)
  6. Income Tax Assessment order
  7. Water bill with current address
  8. Electricity bill with current address
  9. Certificate from reputed employer on letterhead with a seal and signature
  10. Parents’ passport (copy of first and last page)
  • Note
    • The address proof submitted should be of current place of residence only. The passport application form will contain provisions to provide previous places of residence in the year preceding the date of application.
    • Aadhar card or e- Aadhar will be accepted as both Proof of Address (POA) and Proof of Photo Identity (POI) upon successful validation with the Aadhar database.
    • Minor students can submit address proof issued in the name of their parents.
  • Proof of Date of Birth –
  1. Birth certificate issued by the local municipal authority or other competitive authority authorized by the Registrar of Births & Deaths
  2. Secondary school leaving certificate/School leaving certificate. Only certificates issued by recognized boards and educational institutions are valid.

Along with the documents above, students applying for a passport may also be required to provide the documents below.

  • Student ID card
  • Bonafide certificate issued by the college

In case the original educational certificates are not available due to reasons such as certificates being retained by the current institution where the student is studying, a certificate or a letter stating the same can be produced. Photocopies of certificates attested by the head of the institution can be submitted.

Documents required for passport-If you are a Senior Citizen

For first-time applications, if you have furnished your Voter ID, Aadhar card, PAN and an affidavit in the format of Annexure -I, the passport application will be processed on Post-Police Verification basis. This enables faster passport issuance. There will be no additional fees required, but this is subject to online validation of the Aadhar number. PAN and EPIC might also be validated from the respective databases if required.

  • Proof of current address : Applicants should submit any proof of address of her/his current residence only. Irrespective of the duration of stay, and the date from which she/he has been staying there, the present address is required. You are required to mention any other addresses you have resided at for one year prior to the date of application. Aadhar letter, card and e-Aadhar is accepted by the PSK . This is subject to successful validation with the Aadhar database. You may submit any one of the following:
    • Aadhar Card
    • Electricity bill
    • Water bill
    • Telephone bill (landline or post-paid mobile)
    • Proof of Gas connection
    • Voter ID or Election commission photo ID card
    • Income tax assessment order
    • Registered rent agreement
    • Certificate from employer on letter head of reputed companies
    • Bank passbook (regional rural banks, public sector banks, scheduled private sector Indian banks)
  • Proof of date of birth (Any one of the following)
    • Birth certificate issued by a municipal body
    • Secondary school leaving certificate or a certificate issued from a recognized board
    • Affidavit sworn before a notary or magistrate stating place and date of birth as per Annexure ‘A’. This applies especially to illiterate, semi-illiterate, and applicants who have not completed   5th standard.
  • Standard Affidavit as per Annexure -I is mandatory.
  • You may submit any one of the following:
    • Verification Certificate as per the specimen as per Annexure ‘F’
    • Copy of the passport of the child who is staying abroad. A copy of the page bearing the parents’ name is required. The child should be at least 18 years old.
  • Or you can submit any 3 of the following acceptable documents:
    • Voter ID or Electors Photo Identity Card (EPIC)
    • Aadhar Card
    • PAN card
    • Gas Connection Bill
    • Registered Rent Agreement
    • Driving Licenses (valid and within the jurisdiction of State of submission of application)
    • Service Photo Identity Card issued by Central or State Government, Public Sector Undertakings, local bodies or Public Limited Companies
    • SC certificate, ST certificate, OBC certificates
    • Freedom Fighter Identity Cards
    • Property documents such as Pattas, Registered deeds, etc.
    • Pension documents such as ex -service men’s Pension book or Pension Payment order, ex -service men’s Widow or Dependent Certificates, Widow Pension Order, Old Age Pension Order
    • Railway Identity Cards
    • Birth Certificates issued under the RBD Act
    • Student Photo Identity Cards issued by Government Recognized Educational Institutions for full-time courses
    • Bank passbook, Kisan passbook or post office passbook

Senior citizens should follow the same procedures and rules as the general adult category. However, the government has made provisions to give special preferences to the aged population. There are many facilities in place to speed up the process for the older folk so that they don’t have to bear standing in long lines, waiting, and getting delayed.

Documents required for Dual Citizenship

The Indian constitution and existing laws do not allow dual citizenship.

Documents required for Duplicate Passport

The documents which have to be submitted are listed below:

  1. Affidavit with details of how your passport got damaged or was lost (Annexure ‘L’)
  2. No Objection Certificate (Annexure ‘M’)/Prior Intimation Letter (Annexure ‘N’)
  3. Current address (proof)
    • Telephone bill
    • Certificate from Employer
    • Water bill
    • Election Photo ID card
    • Income Tax Assessment Order
    • Passport copy of spouse
    • Aadhar Card
    • Parents’ passport copy
    • Gas Connection
    • Registered Rent Agreement
  4. Police report (FIR)
  5. Date of birth (proof)
    • Secondary School leaving certificate from the last school attended or an educational institution which is recognized
    • Birth certificate issued by a municipal authority or an authorized office
    • Semi-literate or literate applicants: Affidavit sworn before a notary stating the place and date of birth (Annexure ‘A’)
  6. Photocopy of the first and last pages of old passport (ECR/Non-ECR page), if available (optional)
  7. Passport-size photographs

It is important to note that submission of a photocopy of the previous passport is not compulsory. However, pertinent details mentioned in the previous passport such as place of issue, date of issue, passport number and date of expiry should be submitted with the passport application form. In case an individual does not have the aforementioned details, he should contact embassy officials.

Documents required for Re-issue of Passport

 

One would want to reissue a passport for reasons like change in personal details, exhaustion of pages, damage, loss or expiry.

Below is a list of general documents required to apply for a passport reissue, but the PSK or Passport Office may communicate the need for additional documentation depending on the case in question.

  1. Identity proof: A legal and recognized document that contains your full name as it should appear on official records, and as it should appear on your passport.
  2. Age proof: A legal and recognized document that confirms the applicants name as well as her/ his date of birth. A birth certificate serves this purpose.
  3. Address proof: A legal and recognized document that confirms that the applicant actually resides at the address mentioned in the application.
  4. Photographs: Most PSKs are equipped with cameras designed to take passport sized photographs of applicants, but you may be required to produce 2 recent passport sized photographs of yourself.
  5. Old passport booklet: In cases of reissue of passports, the passport office will demand that the applicant submit his / her old /expired / damaged booklet.
  6. Self-attested copy of first and last pages of booklet: The first two and last two pages of the old booklet being submitted along with the application for reissue must be self-attested by the applicant.
  7. Affidavit that states how you lost or damaged your passport.
  8. Police report in original.
  9. Appointment application receipt: If an appointment with the PSK has been booked online, the passport office will require a print out of the final page on the website that confirms the payment, time, and date of the appointment.
