1. Visit the official website of Ndmc. 
2. Enter your Application number/ Registration number/ Date of birth. 
3. Choose the gender. 
4. Enter the father’s and mother’s name. 
5. Select place of birth from the drop down menu.
6. Enter the captcha code and click on “Select”.
7. On the next page, again click on “Select” under “Stage”. 
8. The details of the child will be displayed on the screen. 
9. Below it, enter the applicant’s name, address, email ID and address.
10. Cross check all the details. Click on “Submit”.

If the application gets approved, the applicant can download the birth certificate. The steps for  NDMC birth certificate download are as follows:

1. Fill in the application no and check the details. 
2. Ndmc birth certificate will be shown. 
3. Click on “Print certificate”.  
4. Download the birth certificate or print it.

Ndmc birth certificate name addition is a very easy process. The parents can follow the steps mentioned below for clear understanding. 

1. Visit the official website of Ndmc
2. Enter your Application number/ Registration number/ Date of birth. 
3. Choose the gender. 
4. Enter the father’s and mother’s name. 
5. Select place of birth from the drop down menu.
6. Enter the captcha code and click on “Search”.
7. Fill in the applicant’s mobile number. Click on SMS. 
8. Enter the OTP received on the mentioned phone number. 
9. Mention the relation with children in the next column. 
10. Enter father’s name and aadhar number. 
11. Login to DigiLocker. Upload aadhar card. 
12. The name inclusion page will appear. Enter all the details.
13. Submit the application form and make payment.  
14. Challan number will be displayed on the screen.  Click on “click to pay challan”.
15. Pay the challan amount and download the ndmc birth certificate. 

Note: If an applicant is applying for name inclusion after 365 days, they are required to pay the challan amount.
