How to Add Surname in Indian Passport

If you want to change surname then you need to apply for reissue of passport. You need to submit the documents to fix surname problems in your passport in India. These documents can vary depending on the reason why you want to change surname. (such as marriage, divorce, adoption, etc.) 

• Go to official website of Passport Seva.
• Register or Login to your account.
• Now, you have to click on “Apply for Fresh/Re-issue of Passport” link.
• There you have to fill the form 
• Now select “Change in Existing Personal Particulars” under “Type of Application”.
• Choose reason for your surname change from the menu and enter your new surname in the box
• Upload the scanned copies of documents.
• Pay fee online and now book appointment at PSK or POPSK.
• Print receipt. Now you have to carry it with your original documents and passport to the PSK or POPSK on the day of your appointment.
• When you visit PSK/PoPSK you have to submit your documents for verification. And give your biometrics and photographs. .
• Collect your new passport for PSK or wait for it to be delivered to your address by post.

Alternatively, you can fill out an e-form by downloading it. After that, choose ‘Validate’ and save the file. Then follow the instructions to submit this XML file to schedule an appointment with the passport office.
